CFB2 PROGRAM
 PROGRAM OBJECTIVES AND GOALS
To monitor senior customers in Community Board 11 through daily Telephone Reassurance and Access Services.  Ongoing, regular communication with senior customers is maintained in order to assure that assistance is provided if any status changes occur. Often communication itself provides the human contact necessary to allay any anxieties and feelings of isolation or concerns the customers have. Problems can often be resolved via telephone as case workers can make a needs assessment, status determination and arrange appropriate follow up.
